Health Forms & Immunizations
All new students must submit completed health forms before registering for classes.
Student Health Insurance
All students will be opted into Wellfleet student health insurance for the 2023-24 academic year. If you choose to opt-out, please do so by the July 15th deadline. The waiver form is available on the Wellfleet site.

All students are required to complete health forms in the Student Health Portal. Having this information allows us to better meet your health care needs.
Required Immunizations
All enrolled students must have 2 MMRs or the equivalent, or a positive titre.
